Certain 2022 Ford Maverick pickup trucks could experience an improper air bag deployment that could increase the chance of injury during an accident. Because of this improper deployment, these vehicles fail to meet the Federal Motor Vehicle Safety Standards (FMVSS) for “Ejection Mitigation”.

Ford became aware of the problem in June 2022 when they were contacted by the National Highway Traffic Safety Administration (NHTSA) Office of Vehicle Safety Compliance, regarding the results of a compliance audit on a 2022 model year Maverick. The compliance audit showed that the displacement results for the front lower primary target exceeded the federal regulation of 100 mm. Ford opened an investigation that included comparing their data to the NHTSA test vehicle data, but they were not able to determine the cause of the problem. The cause of the airbag problem is still under investigation.

According to the defect report, trucks included in this action were built with side curtain air bags that may not meet the displacement requirements of FMVSS 226 S4.2.1 for the first row lower primary target location. A vehicle that does not comply with FMVSS 226 increases the risk of occupant injury in a crash. Approximately 65,000 – 2022 model year Ford Maverick vehicles with build dates from February 3, 2021, through July 9, 2022, are affected by this air bag recall.

Owners who receive notices will be instructed to take their Maverick to a Ford dealer to have the left and right side curtain air bag modules replaced with modules of a new design. Ford’s number for this recall is 22C20 and the NHTSA campaign number is 22V-594.

The NHTSA urges all vehicle owners to take immediate action after receiving a notice of a safety defect with their vehicle. Despite recall notices and public warnings, some vehicle and vehicle equipment recalls are never repaired. In 2019 in the United States, there were 966 safety recalls affecting more than 53 million vehicles and motor vehicle equipment. While not all recalls are high profile, all recalls are important.  Automotive Defect Investigation
The Office of Defects Investigation (ODI) of the National Highway Traffic Safety Administration (NHTSA) have opened a Preliminary Evaluation (PE21-010) to investigate complaints of malfunctions with the driver frontal air bags in certain Cadillac, Chevrolet and GMC vehicles.

The ODI has received fifteen consumer complaints about certain vehicles experiencing an airbag warning light illumination and/or a failure of the airbag to deploy during an accident with significant frontal damage. Nine of the consumer complaints say the air bag malfunction indicator lamp (MIL) was illuminated and six of the incidents had significant frontal damage with failure to deploy the airbags.

A TSB, issued in March 2021, addresses air bag MIL illumination accompanied by diagnostic trouble codes B0001-1B or B0012-0D. It also reports rust particles in the connection terminal interface of the driver airbag inflator as the cause of the airbag MIL illumination. Illumination of the airbag MIL under these circumstances may result in a non-deployment of the driver’s airbag during a frontal collision and increased risk of injury to the driver.

ODI have opened this preliminary investigation to determine the scope and severity of the potential problem and to fully assess the potential safety-related issues. If the investigation results in a recall, an estimated 749,312 vehicles could be affected.

Toyota will be contacting the owners of certain 2021-2022 Sequoia and 2021 Tundra trucks because of a problem affecting the power steering gear assembly. Owner notification letters are expected to be sent out on January 22, 2022.

In late September 2021, Toyota observed an increase in field reports and warranty claims affecting the power steering in certain 2021-2022 Sequoia and 2021 Tundra trucks. A high percentage of these vehicles experienced power steering fluid leaks, all from the area of the power steering gear assembly. Manufacturing records showed that these reports mainly involved vehicles produced during the months of August and September 2021. Toyota began recovering additional parts involved in these reports and sent them to the supplier for inspection.

These inspections found that the circlip holding the oil seal and end stopper in position at the end of the power steering rack housing was disengaged from the groove machined in the rack housing and that the oil seal and the end stopper were moved from the correct position. Further investigation of the returned parts revealed that the groove where the circlip is installed was improperly machined leading to an incorrect groove shape.

Based on these findings, the supplier investigated their production process and identified that one of the four pieces of equipment used to machine the groove in the rack housing was not creating a groove of the correct shape. The tool was inspected, it was found that certain teeth on the cutting tool, which creates the groove in the rack housing, were damaged. Based on these results Toyota has decided to conduct a safety recall campaign.

Owners of the affected vehicles will be notified to return to their Toyota dealer to have the power steering gear assembly inspected and replaced if necessary. Toyota’s numbers for this recall are 21TB10 and 21TA10, and the NHTSA campaign number is 21V-920.

During a configuration review at the supplier for an unrelated manufacturing issue, FCA US LLC Product Engineering became aware of an error with an airbag squib driver circuit that identified three internal faults not mapped to a diagnostic trouble code (DTC). An investigation was opened and it was discovered that some 2021 Jeep Grand Cherokee, 2022 Jeep Wagoneer, and 2022 Jeep Grand Wagoneer vehicles may not set a DTC, and the airbag warning indicator may not illuminate.

The problem started in February 2021, when occupant restraint controllers (ORCs) with an incorrect software version were introduced into vehicle production, through to November 1, 2021, when the suspect ORCs were no longer used in vehicle production. Vehicle and supplier production records were used to determine the vehicles included in the recall population.

According to the defect report, “If specific ORC internal faults are active then a DTC will not be set and the airbag warning indicator may not illuminate. The internal faults will disable deployment of both the driver and passenger airbag squib 3 (related to airbag venting post-deployment) and knee airbags. The airbag warning indicator may not illuminate to notify the driver of possible compromised airbag system functionality. Reduced occupant protection in the event of a crash may result in an increased risk of injury to motor vehicle occupants.”

Owners receiving notices will be asked to return to their FCA dealers to have the occupant restraint controller reprogrammed with the correct software version that maps subject internal faults to a DTC. FCA US, LLC’s number for this recall is Y79 and the NHTSA campaign number is 21V-873.

A manufacturing defect affecting airbags will have a small number of 2008-2017 Buick Enclave and 2013-2017 Chevrolet Traverse owners returning to their dealerships for repairs.

In July 2015 the Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE15-027) into ARC Automotive Inc. (ARC) air bag inflators after receiving reports about two injury incidents involving a driver air bag inflator rupture. One incident involved a 2002 Chrysler Town & Country that had a dual-stage air bag inflator and the other involved a 2004 Kia Optima with a single-stage inflator. Both air bag inflators were manufactured by ARC.

During the evaluation the ODI gathered information from ARC about which air bag module manufacturers used the subject ARC inflators and requested information from the identified air bag module manufacturers about which vehicle manufacturers used modules with the subject ARC inflators. That process identified additional affected vehicle manufacturers, including General Motors.

According to the defect report, GM will be recalling certain 2008–2017 Buick Enclave and 2013-2017 Chevrolet Traverse vehicles for front driver airbag inflators which may have been installed either as original equipment or as part of a replacement driver airbag module during service. These airbags could contain a manufacturing defect that may result in an inflator rupture during deployment. If the inflator ruptures it could cause metal fragments to pass through the airbag and into the vehicle, potentially causing injury or death to vehicle occupants.

Dealers will inspect and replace the driver’s air bag module as necessary. GM’s number for this recall is N212349040 and the NHTSA campaign number is 21V-782.

General Motors will be contacting the owners of certain 2020-2022 Cadillac CT4 And CT5 vehicles because of a problem affecting the roof rail side air bags.

In June 2021, GM investigators were contacted by the National Highway Traffic Safety Administration (NHTSA) regarding a right-side roof rail airbag (RRAB) in a 2020 Cadillac CT5 that did not deploy properly during a test for out-of-position occupants. Inspection of NHTSA’s test photos shows a twisted condition between attachment points of the affected RRAB.

In July, a yard hold was initiated at GM’s assembly plant. Inspection of approximately 1,300 vehicles on hold found five vehicles with the twisted RRAB condition. GM is not aware of any accidents or injuries associated with this condition, but have decided to conduct a safety recall.

Dealers will inspect both left and right-side roof rail air bags and reinstall the air bags, as necessary. Owner notification letters are expected to be mailed in September. GM’s number for this recall is N212342780 and the NHTSA campaign number is 21V-611.

In mid-June 2021 General Motors became aware of roof rail airbag (RRAB) inflator ruptures in three 2015 model year Silverado vehicles while the vehicle was unoccupied and not in use. In all three incidents, the steel inflator body sidewall split open, suddenly releasing the gas stored inside the chamber. GM’s Safety and Field Action Decision Authority have decided to conduct a safety recall.

According to the defect report, The RRAB inflators on the left and right side roof rails may contain a manufacturing defect that could result in inflator end cap separation or inflator sidewall split. If this happens, the compressed gas will escape from the inflator and the end cap or other components can be propelled into the vehicle, potentially injuring occupants.

Owners will receive a second notice when the remedy is available. This recall supersedes NHTSA recall number 20V-736. General Motors will be contacting the owners of certain 2021 Chevrolet Traverse and 2021 Buick Enclave SUVs, about a problem that could result in the front air bag not deploying properly. Owner notification letters are expected to be mailed out beginning of August 2021.

According to the defect report, during a weekly service programming system (SPS) audit, it was discovered that a small number of 2021 Chevy and Buick models may have been manufactured with the incorrect sensing diagnostic module (SDM) data calibration. The data files were corrected in the SPS system and the problem was reported to GM’s safety program. Their investigation found a small number of 2021 Chevrolet Traverse and 2021 Buick Enclave vehicles may have received the same incorrect data and decided to conduct a safety recall.
